I told you so is totally a sign that things are wrong and it being said a lot is really signaling to the manager of that team that they need to dig in and figure out what is actually going on. Whenever I've heard it's always someone that knows there is a problem and either can't figure out how to fix it or they don't feel listened to when they've come up with solutions for it and so they've just checked out.
